Medical Network Patients: How to Request a Copy of Your Medical Records:

The easiest way to access your medical records is through the MyChart app. For those who would like a copy of their records mailed, please print and complete the MarinHealth Medical Network Medical Record Release Form (English | Spanish). The release form must be completed, dated and signed submitted with a valid color copy of your photo ID. We ask that you specify what components of your medical records you wish to obtain. Often, the discharge summary, operative report, and/or history and physical report contain relevant information to suit your needs.
