Diabetes Care at the Braden Diabetes Center

The MarinHealth Diabetes Care Program offered through our Braden Diabetes Center, provides a continuum of expert care and resources for diabetes patients, their families, and caregivers. With the proper education, support, and medical supervision, patients can learn to successfully manage their diabetes and live healthier lives.

MarinHealth provides an exceptionally robust and well-planned continuum of expert inpatient and outpatient care for diabetes patients. The program also offers a wealth of helpful resources for patients, families, and caregivers.

  MarinHealth Medical Center has a multidisciplinary inpatient and transition care program to help patients with diabetes maintain control of their blood sugar during their stay and after discharge.
• Braden Diabetes Center
  Our unique outpatient center empowers people with diabetes by providing the tools and techniques they need to live a healthier life with diabetes, one step at a time. Approved by the American Diabetes Association as meeting the National Standards for Diabetes Self-Management Education, the program includes preventive strategies and self-management. Our program is structured to help patients understand diabetes, make healthy choices, and learn to confidently manage their blood sugar in any situation. Education and training are offered in both one-on-one and group settings. The Center also serves as a bridge clinic to help hospitalized patients manage their diabetes after discharge until the next appointment with their primary care provider.
